Health care coverage: fluoride treatments

The law requires qualifying commercial coverage to pay for medically necessary fluoride varnish without patient cost sharing and broadens Medi-Cal access through medical, dental, and public-health settings.

What the law does ​

• Requires qualifying health plans and insurers, beginning January 1, 2027, to cover medically necessary fluoride varnish without deductibles, copayments, coinsurance, or other cost sharing whether billed as dental or medical care.
• Excludes specialized plans and Medicare supplement coverage, and excludes Medi-Cal managed care contracts from the commercial-plan requirement.
• Makes fluoride and other department-defined fluoride treatment a Medi-Cal benefit for children under 21.
• Requires the State Department of Health Care Services to issue billing guidance and update Medi-Cal policy by July 1, 2027.
• Allows Medi-Cal fluoride varnish without site restrictions when provided by enrolled licensed medical or dental providers, or trained and supervised qualified health or public-health professionals.

• Children under 21 enrolled in Medi-Cal.
• Children with qualifying private health-plan or insurance coverage for pediatric oral care.

Context ​

Medi-Cal implementation depends on available federal funding and required federal approvals, while the private-coverage requirement does not apply if state cost defrayal is required and no appropriation is made.
